Always apply memory baselines

I’d like to store a personal context block (e.g., “I’m Jason, 55, in NZ, with a cerebellar tumour history, full truncal ataxia, extreme falls risk, use a recumbent pedal‑assist trike, etc.) that Omi automatically loads for every session and can optionally surface in my daily summary. I don’t want to keep re‑explaining my disability and mobility constraints; I want Omi to always operate with that as baseline context
